A birth certificate is an official government document that comes from the state. A birth record is an unofficial document that St. Luke's Health provides to new parents as a souvenir that can be suitable for framing. Copies of the birth record can be obtained through our Medical Record department. You can contact the department at Baylor St. Luke’s Medical Center by calling (832) 355-2077.

How can I obtain copies of medical records from previous visits to St. Luke's Health?

To protect our patients' privacy, we don't transmit confidential patient information via e-mail. The best way to obtain copies of your records is to call our Medical Record department at (832) 355-2077 during business hours, Monday - Friday (8:00 a.m. - 4:30 p.m.). One of our representatives will be happy to assist you.

To obtain a copy of your medical records, you will need to complete this form. You can deliver the form in person to a St. Luke's Health Medical Records Department or you can mail it. IMPORTANT: There may be a charge for your records, so we strongly encourage you to call and speak to one of our representatives before mailing in the above form.
