The Transitional Care Clinic at The Woodlands Hospital

The Transitional Care Clinic (TCC) team’s goal is to ensure a successful transition from hospital to home. We offer appointments to congestive heart failure patients at St. Luke’s Health–The Woodlands Hospital within seven days of discharge.

An essential component of staying healthy and monitoring your heart failure is to understand your medical condition, learn about your medications, and recognize the importance of a balanced diet.

Support for Heart Failure Patients

During your TCC visit, you receive heart failure education tips on living a healthier lifestyle and a review of current medications and vital signs (blood pressure, weight, etc.). The team also ensures subsequent follow-up visits are scheduled with your physician.
