St. Luke's Health Selects Population Health Advisors as its Consultant for Population Health Services and a Value-based Approach to Care in Greater Houston
St. Luke’s Health (St. Luke’s) is teaming with Population Health Advisors (PHA), a part of Baylor Scott & White Health, to advance its work on creating models and systems of care that will enhance care delivery in the Houston market. St. Luke’s has been transitioning to a value-based approach to care centered on improving quality and managing the health of patient populations while simultaneously reducing costs. By collaborating with PHA, St. Luke’s will benefit from PHA’s expertise and infrastructure as they operationalize population health management and other quality improvement initiatives.
“The strengths of St. Luke’s and PHA complement each other and set the tone for the collaboration necessary for creating and implementing the value-based approach to care we are on track to achieve here in Greater Houston,” said Michael H. Covert, CEO of St. Luke’s Health. “Among these areas of strength are shared faith-based values and leaders who are dedicated to the mission and vision of quality care and population health.”
Baylor Scott & White Health is the largest not-for-profit health system in the state of Texas. Its staff has implemented a population health infrastructure to improve health outcomes that has resulted in reducing medical plan costs for the Baylor Scott & White Health North Texas Division Employee Health Plan.
“Leaders from our two organizations know that effectively delivering on population health management and transitioning to fee-for-value depends on creating a scalable infrastructure with the capacity to support the deployment and measurement of consistent, quality results. We are excited about our upcoming journey together and look forward to everyone’s commitment and support as we launch this project,” said Gary Brock, Executive Vice President, Baylor Scott and White Health.
