St. Luke's Health–Sugar Land Hospital Receives Premier Inc. Award for Reducing Hospital-acquired Conditions and Readmissions
SUGAR LAND, TX (August 22, 2017)—St. Luke's Health–Sugar Land Hospital (Sugar Land Hospital) has been recognized by Premier Inc. for reducing preventable hospital-acquired conditions (HACs) and readmissions, as part of their participation during the federal Partnership for Patients Hospital Engagement Network 2.0 initiative.
Partnership for Patients is a Centers for Medicare and Medicaid Services (CMS) program that works to improve the quality, safety, and affordability of healthcare for all Americans by working with a Hospital Improvement Innovation Network (HIIN), which includes 4,000 hospitals across the county.
This prestigious award is given to hospitals that have made significant strides in meeting program goals. Eleven harm topics were reviewed and finalists represent the top 10 percent of the HIIN hospitals for a total score (= 90th percentile). Facilities must also be making improvements in 30-day readmissions. HIINs work to sustain and accelerate national progress and momentum towards continued harm reduction, help identify solutions already working, and then disseminate them to other hospitals and providers. The eleven harm topics are:
- Adverse drug events (ADE)
- Catheter-associated urinary tract infections (CAUTI)
- Central line-associated blood stream infections (CLABSI)
- Clostridium difficile (CDI) and antibiotic stewardship
- Injuries from falls and immobility
- Pressure ulcers
- Preventable readmissions
- Sepsis and septic shock
- Surgical site infections (SSI)
- Venous thromboembolism (VTE)
- Ventilator-associated events (VAE)
“Participation in this initiative reinforces our commitment to reliably deliver the best care experience to our community,” said Rob Heifner, Sugar Land Hospital president. “We're proud to receive the Premier Partnership for Patients HEN 2.0 Award as we continue down our path of reducing HACs and readmissions.”
Ambitious goals are set for HIINs. Through 2019, the goal is to achieve a 20 percent decrease in overall patient harm and a 12 percent reduction in 30-day hospital readmissions.
“Sugar Land Hospital is a prime example of performance improvement in healthcare,” said Roshni Ghosh MD, vice president of government services, chief medical information officer, Premier. “Through commitment, transparency, trust and engagement they are collaborating with peers to create a culture of safe, reliable and coordinated care. We are constantly amazed by these accomplishments. Premier congratulates everyone at St. Luke's Sugar Land Hospital for their hard work and tremendous achievements.”
