Houston, TX - St. Luke’s Health and its sponsored Clinically Integrated Network (CIN), Texas Health Network (THN), was recently recognized with the Achievement in Patient Care Access award by Devoted Health Plans, for helping the health insurer’s Texas HMO plans achieve a 4.5 star overall rating from the Centers for Medicare and Medicaid Services (CMS) for contract year 2022. St. Luke’s Health specifically contributed to Devoted Health’s 5-star performance on many patient care measures including those related management of diabetes and hypertension.

As a 4.5-star rated health insurance provider offering Medicare Advantage Prescription Drug plan coverage to Texas residents, Devoted Health earned the high rating for its strong performance in moving the needle on difficult quality measures for their members.

THN features 352 primary care providers and 1,699 specialty providers, and is aligned with 16 hospitals including Baylor St. Luke’s Medical Center, the research and teaching hospital for Baylor College of Medicine. Through St. Luke’s Health network of providers throughout Texas, the THN worked together to apply effective clinical strategies for delivering efficient, affordable, and coordinated care to patients of Devoted Health Plans.
Focused on patient satisfaction, quality of care, cost control and provider satisfaction, key measure improvements for Devoted Health patients within St. Luke’s Health CIN include:
- 85 percent of diabetic members were able to bring their blood pressure under control
- 98 percent of diabetic members monitored their kidney health
- 93 percent with high blood pressure, took their medication as directed
“We also need to recognize Karan Lozano, RN and Deborah Dornes, RN who worked tirelessly on engaging these patients in this contract over the phone for care management to help get their chronic conditions under control and better managed,” Lembcke added.
