How can I obtain copies of medical records from previous visits to St. Luke's Health?
To protect our patients' privacy, we don't transmit confidential patient information via e-mail. The best way to obtain copies of your records is to call our Health Information Management Department at the phone numbers listed below. One of our representatives will be happy to assist you.
To obtain a copy of your medical records, you will need to complete this form. You can deliver the form in person to a St. Luke's Health Health Information Management Department located at one of the following hospitals, or you can mail it.
IMPORTANT: There may be a charge for your records, so we strongly encourage you to call and speak to one of our representatives before mailing in the above form.

How can I secure a copy of my child's (or my own) birth certificate?
Birth certificates are available only through the Houston Department of Health and Human Services Bureau of Vital Statistics. The office is located at:

What is the difference between a birth certificate and a birth record?
A birth certificate is an official government document that comes from the state. A birth record is an unofficial document that St. Luke's Health provides to new parents as a souvenir that can be suitable for framing. Copies of the birth record can be obtained through our Health Information Management Department at each hospital.
