Pediatric Bridge Clinic

The Pediatric Bridge Clinic provides a range of primary care services to newly discharged newborns who do not have an established relationship with a pediatrician. In addition to providing care, the Clinic links parents and caregivers with local resources and transitions the baby’s care to a permanent medical home by the age of six months.

Pediatric Bridge Clinic care is available to newborn patients in The Woodlands, Conroe, Magnolia, Spring, and surrounding areas.
